Tumore dell’esofago: l’immunoterapia migliora la sopravvivenza

Roma, 5 giugno 2021 – La combinazione di immunoterapia più chemioterapia e la duplice immunoterapia possono cambiare la pratica clinica nel trattamento in prima linea dei pazienti con tumore dell’esofago in fase avanzata. Lo studio CheckMate -648, presentato oggi al Congresso della Società Americana di Oncologia Clinica (ASCO), costituisce una pietra miliare nella lotta contro questa neoplasia grazie a risultati mai raggiunti finora. Sono stati coinvolti 970 pazienti, colpiti da tumore dell’esofago a cellule squamose avanzato o metastatico e mai trattati in precedenza. Sono stati considerati sia i pazienti con espressione del biomarcatore PD-L1 (maggiore o uguale all’1%) che tutta la popolazione randomizzata.
“Nei primi, la combinazione di nivolumab più chemioterapia – afferma Stefano Cascinu, Primario Unità di Medicina Oncologica IRCCS Ospedale San Raffaele di Milano e Professore di Oncologia Medica presso l’Università Vita-Salute San Raffaele – ha mostrato un vantaggio davvero impressionante in termini di sopravvivenza globale mediana nel confronto con la sola chemioterapia, pari a 15,4 mesi rispetto a 9,1 mesi. Ottimo anche il risultato raggiunto dalla combinazione delle due molecole immuno-oncologiche, nivolumab e ipilimumab, pari a 13,7 mesi rispetto a 9,1 mesi della sola chemioterapia”. Sostanzialmente sovrapponibili i risultati in tutti i pazienti randomizzati, cioè indipendentemente dall’espressione di PD-L1. “In questo gruppo la sopravvivenza globale mediana è stata di 13,2 mesi con la combinazione di nivolumab più chemioterapia e 12,8 mesi con nivolumab e ipilimumab rispetto a 10,7 mesi con la sola chemioterapia. Anche il tasso di risposta oggettiva è migliore con le combinazioni”.
Nel 2020, in Italia, sono stati stimati 2.400 nuovi casi di tumore dell’esofago, in costante aumento. “Circa la metà presenta la malattia già in stadio avanzato al momento della diagnosi – spiega il Prof. Cascinu -. Oggi la chemioterapia è il trattamento standard per questi pazienti, ma la prognosi rimane sfavorevole perché la sopravvivenza non supera i 10 mesi. Da qui l’importanza di individuare nuove opzioni. Il significativo miglioramento clinico in sopravvivenza di questi due regimi di trattamento evidenzia l’impatto dell’immunoterapia sulla gestione della neoplasia e può portare nuove possibilità di cura per pazienti con malattia già in fase avanzata. Inoltre la duplice immunoterapia è il primo trattamento chemio-free a mostrare un beneficio in sopravvivenza in questi pazienti, che spesso sono molto fragili e colpiti anche da altre patologie. Può quindi rappresentare un’alternativa terapeutica efficace in persone che non tollerano la chemioterapia per le condizioni generali di salute compromesse”.
“L’abuso di alcol e l’abitudine al fumo di sigaretta sono strettamente connessi alla forma squamosa del tumore dell’esofago – conclude il Prof. Cascinu –. Ecco perché è importante promuovere campagne di prevenzione per aumentare le diagnosi in fase precoce e sconfiggere la malattia”.
