Solo il 20% dei percorsi diagnostico terapeutici assistenziali (Pdta) contiene informazioni per applicare quanto previsto dalle linee guida internazionali sulla nutrizione clinica. Emerge dal Rapporto Nutrendo 2023 sulle malattie croniche, presentato alla seconda edizione del Forum della Società̀ Italiana di Nutrizione Clinica e Metabolismo (Sinuc). L’edizione 2023 ha prodotto tre nuovi rapporti: nel primo, dal titolo ‘Nutrizione clinica e malattie croniche’, obiettivo era mappare la presenza della nutrizione clinica nei pdta relativi alla presa in carico di pazienti affetti da malattie croniche. Sono stati individuati, tra il 2020 ed il 2021, 145 Pdta o atti formali attuati dalle Regioni. La maggior parte dei documenti interessa le patologie oncologiche dal 2009 al 2019 (54%), seguite dalle patologie neurodegenerative (16%), Bpco (9%), artrite reumatoide (7%) e scompenso cardiaco (7%), malattie infiammatorie croniche intestinali di malattia renale cronica (3%) e lesioni da pressione (1%). “Da questa analisi dettagliata sui Pdta regionali o di atti formali emerge un quadro frammentario e inadeguato per l’applicazione dei principi di nutrizione clinica nella gestione del malato cronico e oncologico” sottolinea in una nota Maurizio Muscaritoli, Presidente Sinuc e ideatore del Forum Nutrendo. “Solo il 20% dei Pdta valutati, infatti, contiene informazioni sufficienti per poter applicare una terapia nutrizionale secondo quanto previsto dalle linee guida internazionali e sono pochi quelli che danno rilevanza allo stato nutrizionale nella gestione del malato. Allo stato attuale, sono molto rari i Pdta e gli atti formali che considerano fondamentale lo stato nutrizionale nella gestione del malato con patologia cronica o neoplastica e lo considerano tale solo per alcune patologie, come le Mici e alcuni tumori”. “Questo atteggiamento culturale è paradossale in quanto è oramai acclarato come la terapia nutrizionale sia una strategia costo-efficace utile per ridurre la morbilità̀ e migliorare i risultati clinici in tutte le patologie croniche e nei tumori”, sottolinea. E’ auspicabile, evidenzia la nota, una rivisitazione degli attuali Pdta oncologici nei quali il tema “malnutrizione” possa assumere un ruolo di centralità e trasversalità come si può evincere dal Piano Oncologico Nazionale 2023-2027, che dedica ampio spazio alla tematica.
