Schizza la spesa farmaceutica ospedaliera in Italia. In base ai consumi di farmaci nei primi mesi del 2023 rilevati da Iqvia, la spesa farmaceutica per acquisti diretti (ospedaliera) nel 2023 oltrepasserà nuovamente il tetto programmato per legge, arrivando – secondo le stime – a circa 13 miliardi di euro, +6% rispetto al 2022. A fronte di un tetto stimato pari a circa 9,7 miliardi di euro (il 7,65% del Fondo sanitario nazionale, come nel 2021 e 2022), quest’aumento porterà a un disavanzo di circa 3,3 miliardi di euro, in crescita del 20% rispetto al 2022. Di questi la metà, ossia circa 1,6 miliardi, dovrà essere ripianata dalle aziende farmaceutiche – ricorda Iqvia – con il sistema del payback. La restante parte sarà pagata dalle singole Regioni in base al loro superamento del budget assegnato. A partire dal 2022, tuttavia, le aziende che hanno già adempiuto al pagamento integrale dell’onere di ripiano per gli anni 2019 e 2020 godranno di una riduzione della quota da pagare, rideterminando il valore del ripiano come se il tetto fosse dell’8% nel 2022 e dell’8,15% nel 2023. Il payback per tali aziende sarà quindi calcolato come se il ripiano complessivo per il 2023 risultasse di circa 1,4 miliardi di euro. Sono esclusi da questi calcoli i farmaci innovativi e gli innovativi oncologici che rientrano in un unico fondo separato del valore di 1,2 miliardi di euro nel 2023. In questo caso, Iqvia non prevede alcuno sforamento, ma anzi un avanzo di circa 300 milioni di euro che, tuttavia, non va a compensare la spesa per acquisti diretti. La spesa convenzionata (ricetta rossa) è prevista in crescita di circa l’1,5% rispetto al 2022, arrivando a circa 8,1 miliardi di euro. Si manterrà all’interno del tetto programmato, generando un avanzo di risorse in aumento rispetto al 2022 (intorno agli 800 milioni, rispetto ai 728 verificati da Aifa per il 2022).
Farmaci: nel 2023 la spesa ospedaliera sale a +6%
