21, settembre 2023 – La malnutrizione peggiora la prognosi dei pazienti oncologici senza metastasi. Il tasso di mortalità riscontrato nei malati di cancro malnutriti o a rischio di malnutrizione è del 65%, contro il 38% di quelli classificati ben nutriti. Lo stato di malnutrizione ha ridotto la sopravvivenza nei pazienti senza metastasi ma non in quelli con metastasi per i quali non si è dimostrata determinante. Questo il risultato di uno studio tutto italiano, ribattezzato Nutrionco e pubblicato sulla rivista Cancers. Il lavoro, coordinato dal Professor Maurizio Muscaritoli, presidente della Società Italiana di Nutrizione Clinica e Metabolismo (Sinuc), ha coinvolto diversi centri e università italiane, tra cui l’Università La Sapienza di Roma, l’IRCCS Sacro Cuore Don Calabria di Negrar, la Fondazione Policlinico Universitario A. Gemelli IRCCS e l’Ospedale Cardarelli di Napoli. “Nelle persone con cancro la malnutrizione è la faccia che compare su entrambi i lati della medaglia – spiega Muscaritoli – perché può peggiorare la malattia o essere causata dal cancro. La malattia, infatti, ha un metabolismo accelerato e per crescere ‘si nutre’ dell’organismo, sottraendogli proteine e carboidrati. I trattamenti a loro volta, possono determinare anoressia e ridotto introito calorico”. La cachessia è invece considerata una sindrome paraneoplastica, condizione in cui si assiste ad una grave perdita di massa muscolare, con o senza perdita di tessuto adiposo, infiammazione sistemica, astenia e affaticamento. “Una perdita di peso superiore al 5% in 6 mesi o un BMI inferiore a 20 con una perdita di peso superiore al 2% sono i criteri per porre una diagnosi di cachessia”, aggiunge lo specialista. Quindi, in un soggetto di 80 chilogrammi basta una perdita di peso involontaria di 4-5 chili e in una donna di 60 chilogrammi basta un calo di 3 per accendere una spia di attenzione. “Se il paziente non riesce a nutrirsi e perde massa magra si parla di ‘sarcopenia’ ossia la perdita involontaria di massa e performance muscolare, presente nel 20-70% dei pazienti oncologici”, aggiunge Muscaritoli. “Abbiamo riscontrato una probabilità di sopravvivenza globale più elevata nella popolazione generale dello studio ben nutrita rispetto ai pazienti malnutriti”, riferisce il presidente della Sinuc. “Di contro la malnutrizione riduce la probabilità di sopravvivenza nei pazienti con tumore non metastatico”, conclude.
