Milano, 28 giugno 2022 – La Regione Lombardia invierà quanto prima una nota ai Direttori Generali per sollecitare ulteriormente la somministrazione della combinazione di anticorpi monoclonali per la profilassi del Covid-19 alle categorie di pazienti fragili indicate da AIFA. Finora sono state distribuite nelle diverse Regioni circa 20 mila dosi, ma le prime 10 mila scadranno a fine luglio e rischiano di restare in gran parte inutilizzate nelle farmacie ospedaliere (per le altre 10mila il termine ultimo è dicembre). La Regione Lombardia è stata la più virtuosa a livello nazionale nella prescrizione della combinazione di monoclonali, avendo reclutato il più alto numero di pazienti, ma serve una ulteriore accelerazione con il richiamo dei pazienti fragili, per evitare che le fiale disponibili non siano utilizzate in tempo. È quanto emerso nel recente incontro virtuale che ha interessato la Lombardia e che è parte del tour in 10 Regioni, promosso da Senior Italia FederAnziani in collaborazione con AstraZeneca.
“La combinazione dei due anticorpi monoclonali umani, tixagevimab e cilgavimab, rappresenta un’importante opzione in grado di prevenire la malattia nei pazienti che hanno risposto in modo insufficiente al vaccino – afferma Giuliano Rizzardini, Direttore Responsabile Malattie Infettive 1 all’Ospedale Luigi Sacco di Milano -. Una singola dose, facilmente somministrabile per via intramuscolare, determina una protezione duratura, per almeno 6 mesi. Nello studio pubblicato sul ‘New England Journal of Medicine’, la combinazione ha ridotto dell’83% il rischio di sviluppare il Covid sintomatico. Però finora il suo utilizzo nel nostro Paese è stato insufficiente, nonostante i pazienti fragili ed immunodepressi siano molto numerosi ed i numeri della pandemia non stiano diminuendo nemmeno nel periodo estivo”.
“Diversi ostacoli hanno limitato finora l’uso di questo farmaco per la prevenzione della malattia Covid sintomatica – spiega Matteo Corradin, Dirigente Unità Operativa Polo Ospedaliero della Regione Lombardia -. A partire dal vincolo costituito da un test sierologico negativo, rimosso da AIFA anche a seguito di nostre sollecitazioni. Ora è necessaria un’ulteriore accelerazione per somministrare il farmaco ai pazienti che richiedono una protezione aggiuntiva al vaccino. Invieremo quanto prima una nota ai Direttori Generali per sollecitarli nuovamente all’utilizzo della combinazione di monoclonali per la profilassi del Covid”.
AIFA ha stabilito di lasciare al clinico la decisione sul trattamento con questa opzione farmacologica, in base alla valutazione del singolo paziente. “Nel nostro Paese restano ancora troppe persone le cui condizioni individuali non permettono una valida risposta protettiva alla vaccinazione – conclude Roberto Messina, Presidente Senior Italia FederAnziani -. Si stima che siano oltre 150mila i pazienti potenzialmente candidabili al trattamento con la combinazione di monoclonali. Si tratta, in particolare, dei pazienti trapiantati, affetti da patologie onco-ematologiche, in trattamento chemioterapico attivo, oppure con farmaci immunosoppressori per patologie ad esempio reumatologiche o neurologiche, o colpiti da immunodeficienze primarie. Non possiamo lasciarli soli”.
